The specimen, cut across just above the ankle joint, is viewed from below so that the side-to-side relations of the structures that have been followed through the preceding sections of the foot remain unchanged.

1. Extensor digitorum longus muscle
2. Anterior lateral malleolar artery
3. Anterior tibiofibular ligament
4. Interosseous tibiofibular ligament (continuous above with the interosseous membrane of leg)
5. Fibula
6. Posterior tibiofibular ligament
7. Peroneus longus muscle (tendon)
8. Peroneus brevis muscle
9. Flexor hallucis longus muscle
10. Lesser saphenous vein
11. Deep transverse septum
12. Tendo calcaneus
13. Extensor hallucis longus muscle
14. Tibialis anterior muscle
15. Superior extensor retinaculum
16. Greater saphenous vein
17. Tibia
18. Tibialis posterior muscle (tendon within synovial sheath surrounded by flexor retinaculum)
