In recent years, and with the advent of pyrosequencing, there has been a virtual explosion in the number of newly identified astrovirus genotypes. With this has come an increased understanding in astrovirus biology, structure, epidemiology, immunology, and disease pathogenesis including the likelihood of extraintestinal and systemic infections. Specifically, the dissertation investigates virus transfer between surfaces and virus recovery from surfaces, models human-fomites interactions to estimate exposure and infection risk, and elucidates causal links between microbial contamination and illness in child care centers. Indirect contact transmission refers to person-to-person transmission of disease via an intermediate fomite (e.g., inanimate object acting as a carrier of infectious disease). The role of indirect contact in disease spread is poorly understood in part because the transmission route of viral pathogens is often difficult to determine. Transmission of respiratory and gastrointestinal viruses can occur through multiple routes (e.g., direct contact, indirect contact, airborne, and common vehicle), and the relative contribution of each route to total disease burden is unclear. The first study in this dissertation examines virus transfer between skin and surfaces, a necessary step in fomite-mediated transmission of viral disease. In the study, transfer of virus between fingerpads and fomites is explored in a laboratory setting. Bacteriophage (fr, MS2, and PHIX174) are used as proxies for pathogenic virus, and over 650 unique transfer events are collected from 20 different volunteers. The study concludes that approximately one quarter (23%) of recoverable virus is readily transferred from a contaminated surface (e.g., a fomite) to an uncontaminated surface (e.g., a finger) on contact. Using the large data set, the direction of transfer (from fingerpads-to-fomite or fomite-to-fingerpad) and virus species are demonstrated to both significantly influence the fraction of virus transferred by approximately 2-5%. To investigate the relative importance of factors contributing to fomite-mediated transmission, a child's risk of illness from exposure to a contaminated fomite is modeled. Specifically, the model estimates a child's exposure to rotavirus using a stochastic-mechanistic framework. Simulations of a child's contacts with the fomite include intermittent fomite-mouth, hand-mouth, and hand-fomite contacts based on activities of a typical child under six years of age. In addition to frequency of contact data, parameters estimated for use in the model include virus concentration on surface; virus inactivation rates on hands and the fomite; virus transfer between hands, fomite, and the child's mouth; and the surface area of objects and hands in contact. From the model, we conclude that a child's median ingested dose from interacting with a rotavirus-contaminated ball ranges from 2 to 1,000 virus over a period of one hour, with a median value of 42 virus. These results were heavily influenced by selected values of model parameters, most notably, the concentration of rotavirus on fomite, frequency of fomite-mouth contacts, frequency of hand-mouth contacts, and virus transferred from fomite to mouth. The model demonstrated that mouthing of fomite is the primary exposure route, with hand mouthing contributions accounting for less than one-fifth of the child's dose over the first 10 minutes of interaction. Based on the findings from the model that concentration of virus on a fomite influences a child's risk of illness, we investigate methods to recover virus from fomites. In a literature review and subsequent meta analysis, we demonstrate that the outcome currently used to describe virus contamination, positivity rate, is biased by the authors' selected sampling methods. We follow up, in the laboratory, with a comparison of the identified methods and demonstrate that polyester-tipped swabs prewetted in 1/4-strength Ringer's solution or saline solution is the most efficient sampling method for virus recovery tested. The recommended method is compatible with plaque assay and quantitative reverse-transcription polymerase chain reaction, two techniques used to quantify virus. The link between hand / fomite contamination and infection risk was explored in a field study at two child care centers over four months. Both respiratory and gastrointestinal disease incidence were tracked daily, while hand and environmental surface contamination were monitored weekly between February 2009 and June 2009. Microbial contamination was determined using quantitative densities of fecal indicator bacteria (e.g. Escherichia coli, enterococci, and fecal coliform) on hands and fomites as well as presence/absence of viral pathogens (e.g. enterovirus and norovirus). Health was monitored daily by childcare staff, who tracked absences, illness-related absences, and symptomatic respiratory and gastrointestinal illness. The resultant data suggests that increases in microbial contamination led to increases in symptomatic respiratory illness four to six days later, in agreement with typical incubation periods for respiratory illness. Similarly, respiratory illness led to increases in microbial contamination on hands during presentation of symptoms, and on fomites in the following three days.

Spx of Bacillus subtilis has been extensively studied, but very little has been published about it. This book incorporates a number of studies that have been conducted in other Gram positive bacteria, which examined the role of Spx orthologs in stress response, bacterial development and virulence. The book contains an overview that will introduce the protein and its orthologous forms, its association with RNA polymerase, the species of Gram-positive bacteria in which it is found, and the conditions in which it is abundant and active. Spx is a member of a large group of proteins belonging to the ArsC/Spx protein family, so the review touches upon the bioinformatic support for the protein family composition and its meaning with regard to protein structure/function.
- 2010Hutton, David William; Brandeau, Margaret L.; Owens, Douglas K.; So, Samuel.Hepatitis B (HBV) is a vaccine-preventable viral disease that, if untreated, can lead to death from liver disease in 25 percent of patients. Infection with HBV is a major global public health problem, particularly in Asian populations. In an era of limited healthcare budgets, mathematical models can be useful tools to identify cost-effective programs and to support policymakers in making informed decisions. This dissertation describes research on public health policies related to screening, vaccination, and treatment for HBV. It also describe contributions to the theoretical literature on when to stop catch-up vaccination for chronic infectious diseases. In the United States as many as 10% of Asian and Pacific Islander adults are chronically infected with HBV, and up to two thirds are unaware that they are infected. Using Markov models of infection, treatment and disease, I find that screening programs for HBV among Asian and Pacific Islander adults are likely to be cost effective and have clinically significant benefits from identifying chronically infected persons for medical management. Liver disease associated with childhood-acquired HBV is a leading cause of death among adults in China. Approximately 20% of children under age 5 years and 40% of children aged 5 to 19 years remain unprotected from HBV. Using a Markov model of infection and disease progression I find that HBV catch-up vaccination for children and adolescents in China would improve the health of the population and save costs over the long term. Although the 20th century has seen incredible development of safe and effective vaccines, many people remain susceptible to vaccine-preventable diseases. "Catch-up vaccination" for age groups beyond infancy can be an attractive and effective means of immunizing people who were missed earlier. However, as vaccination rates increase, catch-up vaccination may become less attractive. This chapter addresses the question of when to discontinue catch-up vaccination programs as immunization rates increase. I use a cost-effectiveness framework: I consider the cost per quality-adjusted life year gained of catch-up vaccination efforts, as a function of immunization rates over time and consequent disease prevalence and incidence. I illustrate the results with the example of HBV catch-up vaccination in China. I contrast results from a dynamic modeling approach with an approach that ignores the impact of vaccination on disease incidence.

At first, it was considered a revolutionary biomedical procedure, which could potentially cure any disease for which the molecular bases were understood. Since then, gene therapy has gone through many stages and has evolved from a nearly unrealistic perspective to a real life application. Clinical efficacy in humans was demonstrated at the beginning of this century after its successful application in small-scale clinical trials to cure severe immunodeficiency in children. However, their successes were overshadowed some time later by the occurrence of vector-related leukaemia in a number of treated children. It is in this context that lentiviral vectors have appeared, with improved efficiency and, possibly, increased biosafety. Very recently, the first clinical trials with lentivectors have been carried out with some success. This Brief firstly defines gene therapy, and places lentivectors within this fascinating therapeutic strategy. Recent years have seen an exponential growth in knowledge about the molecular and cellular mechanisms of antibody activity. These new results dramatically changed our view of how antibodies function in vivo. The importance of this class of molecules is demonstrated by the heightened susceptibility to infections of humans and mice with an altered capacity to generate pathogen specific antibody responses. Thus, the majority of our currently available vaccines, such as vaccines against influenza, measles and hepatitis focus on the generation of long lasting antibody responses. Recent evidence from a variety of in vivo model systems and from human patient cohorts has highlighted the exclusive role of cellular Fc-receptors for certain immunoglobulin isotypes and subclasses. With the recent discovery of a human Fc-receptor for IgM all different human immunoglobulin isotypes now have a cellular receptor, providing a feedback mechanism and link between antibodies and the cellular components of the immune system. Moreover it has become clear the complement and Fc-receptor system are tightly connected and regulate each other to ensure a well balanced immune response. Among the immunoglobulin isotypes IgG plays a very important protective role against microbial infections and also as a therapeutic agent to kill tumor cells or autoantibody producing B cells in autoimmune disease. Transfer of our knowledge about the crucial function of Fc-receptors has led to the production of a second generation of therapeutic antibodies with enhanced binding to this class of receptors. Binding of antibodies to Fc-receptors leads to the recruitment of the potent pro-inflammatory effector functions of cells from the innate immune system. Hence, Fc-receptors link the innate and adaptive immune system, emphasizing the importance of both arms of the immune system and their crosstalk during anti-microbial immune responses. Besides this pro-inflammatory activity immunoglobulin G (IgG) molecules are long known to also have an anti-inflammatory function. This is demonstrated by the use of high dose intravenous immunoglobulins as a therapeutic agent in many human autoimmune diseases. During the past five years several new insights into the molecular and cellular pathways of this anti-inflammatory activity were gained radically changing our view of IgG function in vivo. Several lines of evidence suggest that the sugar moiety attached to the IgG molecule is responsible for these opposing activities and may be seen as a molecular switch enabling the immune system to change IgG function from a pro- to an anti-inflammatory activity. There is convincing evidence in mice and humans that aberrant IgG glycosylation could be an important new pathway for understanding the impaired antibody activity during autoimmune disease. Besides this tremendous increase in basic knowledge about factors influencing immunoglobulin activity the book will also provide insights into how these new insights might help to generate novel therapeutic approaches to enhance IgG activity for tumor therapy on the one hand, and how to block the self-destructive activity of IgG autoantibodies during autoimmune disease on the other hand.

A.The second volume on One Health explains in detail how to implement three key aspects of the One Health paradigm food safety and security, national plans for a holistic one health approach, and relevant new technologies and approaches. The fourteen chapters, each by an internationally recognized authority, are organized into three sections of four or five chapters each, that break new ground in clarifying precisely how One Health can become an operational reality in local, national and international public health initiatives. Section three begins with an international overview on food safety and security. The importance of a One Heath approach is then explained in relation to four specific problems?the growing prevalence of antibiotic resistance, bovine spongiform encephalopathy and pathogenic E.coli. Section four begins with the role of the Food and Agricultural Organization (FAO) in promoting One Health.
