This book provides recommendations for evaluation and therapy in the area of acute pediatric neurology; these are presented didactically with frequent use of illustrations and algorithms. Chapters in the first part of the book discuss presenting symptoms of acute neurological conditions. The second part of the book covers major areas of acute pediatric neurology and each of these chapters has three key elements: description of presenting symptoms; recommended assessments; and recommended interventions. Acute Pediatric Neurology provides an accessible, clinically focused guide to assist physicians in the emergency ward or intensive care unit in decisions on diagnosis and therapeutic interventions in all major acute pediatric neurological diseases.

"During the past several decades, interest in children's psychological disorders has grown steadily within the research community, resulting in a burgeoning knowledge base. The majority of the attention and funding, not surprisingly, has focused on the more prevalent and well-known conditions. Although this raises the odds that young people with more well-known disorders such as ADHD, autism, and learning disorders will receive much-needed professional assessment and intervention, children with less frequently encountered disorders may experience a higher risk of misdiagnosis and inappropriate treatment"--Back cover.

Empirical research has clearly demonstrated that animal abuse in childhood is associated with family violence and violent behavior towards humans in general. Such abuse is accordingly of increasing interest within human services and the criminal justice system. This handbook will serve as an ideal resource for therapists in social work, psychology, psychiatry, and allied fields who work with children who have abused animals. It provides step-by-step guidance on how to assess, develop appropriate treatment plans for, and treat children who commit animal abuse, based on the AniCare model developed by the Animals and Society Institute. Exercises cover the identification and expression of feelings, the development of empathy, self-management skills, and working with parents. Careful consideration is also paid to the effects of witnessing animal abuse. The theoretical framework is eclectic, encompassing cognitive behavioral, psychodynamic, and attachment theories. A number of illustrative case studies are included, along with excerpts from treatment sessions. Accompanying electronic supplementary material demonstrates role-played assessment and treatment and includes workshop presentations of pedagogic material.

This richly illustrated book presents the pediatric applications of PET/CT in the full range of scenarios frequently encountered in a professional setting. It opens with a thorough introduction covering the fundamental science and the clinical basis for use of PET/CT in this age group. Pitfalls and artifacts are examined, and normal variations and benign findings are carefully described. Each subsequent chapter addresses the role of PET/CT with different radiopharmaceuticals in the evaluation and management of a specific disease. The full range of oncological diseases is covered, including the rare ones. Succinct descriptions of clinical cases are included and, when appropriate, comparisons are made with other modalities. In addition, the role of PET/CT in biopsy guidance and in radiation therapy planning is explained. This book will be invaluable for residents and practitioners in nuclear medicine, radiology, oncology, radiation oncology, and nuclear medicine technology

Atopic dermatitis is amongst the most common disorders of the skin in young people around the world. This book presents the newest insights and novel concepts in relation to all aspects of this skin disorder. Classification, clinical features and differential diagnosis are reviewed in the first chapter, while risk factors, susceptibility and an epidemiological overview are outlined in the next chapter, followed by a detailed discussion of the immunology of the skin and the pathophysiology of dermatitis. Further chapters present new data delineating clinical aspects of current genetic findings and insights into concepts of neuroimmunology, as well as the very crucial and severe clinical aspect of itching. New data on the topic of inhalant allergy, specific immunologic treatments and the latest on food allergy are also included.

"Attention-Deficit Hyperactivity Disorder (ADHD) is a chronic neurobehavioral disorder characterized by persistent and often acute distractibility, hyperactivity, and impulsivity. It is a condition usually associated with children but in recent years the diagnosis of ADHD in adults has risen significantly. ADHD often coexists with a wide array of other psychiatric illnesses, including depression and bipolar disorder, thus complicating its assessment and management. In Attention-Deficit Hyperactivity Disorder in Adults and Children, a team of world renowned experts bring together the recent research in this area and cover the history, diagnosis, epidemiology, comorbidity, neuroimaging, and a full spectrum of clinical options for the management of ADHD. The wide ranging, detailed coverage in this text will be of interest to psychiatrists, psychologists, social workers, coaches, physicians, or anyone who wants to develop a deeper understanding of the etiology, characteristics, developmental process, diagnostics, and range of treatment modalities"--Provided by publisher.

Many children with cerebral palsy walk with excessive knee flexion, an inefficient locomotion pattern known as crouch gait that progressively worsens over time without intervention. Patients often receive surgeries to lengthen tight or spastic muscles or to correct bone malalignments, but outcomes are variable and unpredictable. The goals of this dissertation were, first, to objectively identify biomechanical factors that cause a patient to walk with excess knee flexion and, second, to use these factors to predict whether a patient's crouch gait will improve after receiving treatment. We first determined the influence of abnormal bone geometry and crouched gait postures on the function of muscles during walking using a three-dimensional model of the musculoskeletal system. Our analysis revealed that a tibial torsion deformity, a common bone malalignment, reduces the capacity of lower limb muscles to generate extension of the knee and hip joints. Excess tibial torsion may thus be a significant contributor to crouch gait and warrant surgical correction. In addition, our analysis showed that a crouched gait posture markedly reduces the capacity of most lower limb muscles to extend the knee and hip joints. These findings help explain the altered muscle activations, increased energy requirements, and increased joint loading associated with walking in a crouch gait. We also developed a multi-variable linear regression model to estimate how much subjects' crouch gait would change between hospital visits by retrospectively analyzing patients' gait kinematics over time and surgical histories. The regression model was able to explain 49% of the variance in the change in knee flexion between gait visits for the 353 subject-limbs that we analyzed. Further, the regression model classified subject-limbs as 'Improved' or 'Unimproved' with approximately 70% accuracy, which contrasts with the observed improvement rate of 48% among subject-limbs in the study. We further demonstrated that more improvement in crouch gait was expected when subjects had i) adequate hamstrings lengths and velocities, ii) normal tibial torsion, and iii) greater extensor muscle strength, three variables drawn from our knowledge about the biomechanics of crouch gait. This work establishes a new framework--combining biomechanical modeling and statistical analysis--for understanding gait pathology and objectively planning treatment.

This book addresses the unique healthcare needs of adults with chronic childhood illnesses. It presents a model of primary and secondary prevention for emerging adulthood primary prevention in which all young adults are screened for high-risk behaviors and health needs and secondary prevention in which young adults with chronic childhood conditions are optimized through coordinated care, connections to community resources and social/family support. This book is organized in five parts. Part I provides a detailed overview of the health care transition from pediatrics to adult medicine from both a policy and practice perspective. In Part II, the concept of emerging adulthood as a developmental period is explored and strategies for providing improved comprehensive care for this age group are discussed. Part III reviews specific chronic childhood conditions, such as attention-deficit/hyperactivity disorder, autism, cystic fibrosis, and diabetes mellitus, and offers clinical cases and summary reports that can be used as a quick guides to each condition. In Part IV, additional clinical considerations that are not necessarily condition-specific but are highly relevant to the care of young adults with chronic childhood conditions are examined. Part V describes the socio-legal issues involved in caring for this population. Care of Adults with Chronic Childhood Conditions provides primary care providers with a new framework for the care of young adults and identifies opportunities to influence patient health outcomes over a life trajectory.

"The field of critical care medicine is in the midst of a dramatic change. Technological and scientific advances during the last decade have resulted in a fundamental change in the way we view disease processes, such as sepsis, shock, acute lung injury, and traumatic brain injury. Pediatric intensivists have been both witness to and active participants in bringing about these changes and it is imperative that all physicians caring for critically ill children in this new era have a thorough understanding of the applicability of these modern developments to the care of patients at the bedside and to keep up with the rapidly evolving field of critical care medicine. Neurologic emergencies are a common reason for admission to the pediatric intensive care unit (PICU). A thorough understanding of the diseases and disorders affecting the pediatric central nervous system is vital for any physician or health care provider working in the PICU. The Central Nervous System in Pediatric Critical Illness and Injury has been written by an international panel of experts to provide readers with an in-depth discussion on the resuscitation, stabilization, and ongoing care of the critically ill or injured child with central nervous system dysfunction. This book is therefore absolutely crucial for anyone working in the PICU, from physicians, residents and fellows in critical care, neurology, and pediatrics to specialist nurses and support personnel on the PICU and all medical professionals who are involved in the management of these patients."--Publisher's website.

This is a thought-provoking book which introduces a new way of thinking on the development and use of interventions. Relevant to current practice, it advocates early, targeted activity that is focused on increasing muscle activation, training basic actions and minimizing (or preventing) mal-adaptive changes to muscle morphology and function. The authors present recent scientific findings in brain science, movement sciences (developmental biomechanics, motor control mechanisms, motor learning, exercise science) and muscle biology. This knowledge provides the rationale for active intervention, underpinning the need for an early referral to appropriate services. The book features methods for promoting relatively intensive physical activity in young infants without placing a burden on parents which include assistive technologies such as robotics, electronic bilateral limb trainers and baby treadmills. This book begins by specifying the guidelines for training and exercise, outlining the rationale for such intervention. It goes on to cover the fundamentals of neuromotor plasticity and the development and negative effects of limited motor activity on brain organization and corticospinal tract development. Neuromuscular adaptations to impairments and inactivity are discussed along with the General Movement assessment that can provide early diagnosis and prognosis, facilitating very early referral from paediatric specialists to training programs. The book ends with a section featuring various methods of training with the emphasis on preventing/minimizing muscle contracture, stimulating biomechanically critical muscle activity and joint movement.

During the past decade, significant advances have been made in the field of neurodevelopmental disorders, resulting in a considerable impact on conceptualization, diagnostics, and practice. The second edition of Child Neuropsychology: Assessment and Interventions for Neurodevelopmental Disorders brings readers up to speed clearly and authoritatively, offering the latest information on neuroimaging technologies, individual disorders, and effective treatment of children and adolescents. Starting with the basics of clinical child neuropsychology and functional anatomy, the authors present a transactional framework for assessment, diagnosis, and intervention. The book carefully links structure and function - and behavioral and biological science - for a more nuanced understanding of brain development and of pathologies as varied as pervasive developmental disorders, learning disabilities, neuromotor dysfunction, seizure disorders, and childhood cancers. This volume features a range of salient features valuable to students as well as novice and seasoned practitioners alike, including: overview chapters that discuss the effects of biogenic and environmental factors on neurological functioning; new emphasis on multicultural/cross-cultural aspects of neuropsychology and assessment; brand new chapters on interpretation, neuropsychological assessment process, and report writing; an integrative model of neurological, neuroradiological, and psychological assessment and diagnosis; balanced coverage of behavioral, pharmacological, and educational approaches to treatment; case studies illustrating typical and distinctive presentations and successful diagnosis, treatment planning, and intervention; and important practice updates, including the new HIPAA regulations. Child Neuropsychology, 2nd Edition, is vital reading for school, clinical child, and counseling psychologists as well as neuropsychologists.

Cleft Lip and Palate: Diagnosis and Management is an unparalleled review of treatment concepts in all areas of cleft involvement presented by an international team of experienced clinicians. A unique feature of the book is that it largely consists of longitudinal facial and palatal growth studies of dental casts, photographs, panorexes, and cephalographs from birth to adolescence. Throughout the discussion of growth and treatment concepts, the importance of differential diagnosis in treatment planning is underscored. The underlying argument is that all of the treatment goals - good speech, facial aesthetics, dental occlusion, and psychological development - may be realized without the need to sacrifice one for another. In this third edition, further successful physiological treatment protocols are considered and the occurrence and prevention of relapses after some surgical treatments are discussed; again, all solutions are backed up by follow-up records. Additional topics not previously covered include strategies for coping with psychological effects on patients and intra-team conflict, the excellent clinical work being undertaken in Asia, and future multicenter palatal growth studies. It is the editor's hope that, by drawing on the wealth of experience contained in the book, clinicians will be better able to evaluate current treatment practices and concepts and to improve planning of their own treatment procedures.

"As the knowledge base of child clinical and pediatric assessment continues to grow rapidly, so does the need for up-to-date information, explanations, and reference -- especially now, as commonly used tests are revised and reevaluated regularly, and evidence-based assessment has become the standard for practice. The Third Edition of Clinical Assessment of Child and Adolescent Personality and Behavior is an ideal update, designed to help graduate students and practitioners meet the challenges of assessing -- and improving services to -- these young clients. This highly accessible research-to-practice text offers the depth of coverage necessary to understand the field. It is firmly rooted in current knowledge on psychopathology and normative development. The authors review the various types of measures used to assess children's emotion, behavior, and personality, and they provide clear recommendations for their use. In addition, they emphasize skills, from rapport building to report writing, that are crucial to clinical expertise regardless of theoretical background. Finally, the authors provide clear guidelines for conducting evidence-based assessments of some of the most common forms of psychopathology experienced by children and adolescents."--Publisher's website.

"Conduct problems, particularly oppositional defiant disorder (ODD) and conduct disorder (CD), are the most common mental health problems affecting children and adolescents. The consequences to individuals, families, and schools may be severe and long-lasting. To ameliorate negative outcomes and ensure the most effective treatment for aggressive and antisocial youth, early diagnosis and evidence-based interventions are essential. Clinical Handbook of Assessing and Treating Conduct Problems in Youth provides readers with both a solid grounding in theory and a comprehensive examination of the evidence-based assessment strategies and therapeutic practices that can be used to treat a highly diverse population with a wide range of conduct problems. It provides professional readers with an array of evidence-based interventions, both universal and targeted, that can be implemented to improve behavioral and social outcomes in children and adolescents. This expertly written resource: Lays the foundation for understanding conduct problems in youth, including epidemiology, etiology, and biological, familial, and contextual risk factors.Details the assessment process, with in-depth attention to tools, strategies, and differential diagnosis.Reviews nine major treatment protocols, including Parent-Child Interaction Therapy (PCIT), multisystemic therapy (MST) for adolescents, school-based group approaches, residential treatment, and pharmacotherapy.Critiques the current generation of prevention programs for at-risk youth.Explores salient issues in working effectively with minority youth.Offers methods for evaluating intervention programs, starting with cost analysis. This volume serves as a one-stop reference for all professionals who seek a solid grounding in theory as well as those who need access to evidence-based assessment and therapies for conduct problems. It is a must-have volume for anyone working with at-risk children, including clinical child, school, and developmental psychologists; forensic psychologists; social workers; school counselors and allied professionals; and medical and psychiatric practitioners."--Book description (Amazon.com)

Contemporary psychodynamic theory profoundly impacts our understanding of the development of psychopathology in young patients. However, while much has been written about this theory's influence on the psychotherapy techniques used with adults, little attention has been paid to its practical clinical applications in psychotherapeutic work with children and adolescents. This book explains how the new concepts derived from contemporary psychodynamic theory have made it necessary to revise our understanding of and approach to young patients in psychotherapy. It reviews the recent contributions from contemporary two-person relational psychodynamic theory and employs detailed case examples to bring to life this theory's practical applications in child and adolescent psychotherapy. Psychotherapists and students of psychotherapy will find this book a valuable source of information on contemporary psychodynamic theory and a useful resource for introducing a contemporary style into their practice, co-constructing with the patient a narrative to achieve the desired goals.

"AHRQ examines features needed in pediatric electronic health records (EHRs) to ensure quality care for children. The report scanned scientific literature and gathered expert consensus on EHRs used in the care of children. Child health care providers stated that they need access to EHRs with special features to assure the delivery of quality care to pediatric patients. These features relate to a child's evolving physiology, maturity and associated conditions not found in EHRs used for adult patients. Key EHR functionalities for consideration include the ability to build and maintain vaccination records, record growth and development data, calculate weight and age-based medication dosing, manage pediatric diseases, identify pediatric norms such as developmental milestones, and document the relationship between pediatric patients and their parents and caregivers."--Publisher.

Crib death or sudden infant death syndrome is surely among the worst emotional tragedies that any parent can experience. It is the most frequent death-causing syndrome during the first year of life, striking one infant in every 700-1,000. Crib death is a great enigma, and one of the main open issues in the social-medical and scientific setting of contemporary medicine. Despite a wide spectrum of theories and years of research, its etiology remains unclear. The scarcity of knowledge in the field of crib death and its pathology has led to continued and growing concern, prompting an expanding search for explanation that will permit either prediction or rapid diagnosis. This book describes systematic studies of the cardiovascular system and autonomic nervous system carried out in a large number of infants, newborns, and fetuses who have died suddenly and unexpectedly, as well as in age-matched control cases. The cardiovascular and neuropathological findings are presented in detail, and the relationship between crib death and unexplained perinatal death is discussed. This monograph will aid pathologists, forensic pathologists, pediatricians, obstetricians, and neonatologists in recognizing all potential morphological substrata. It puts forward a well-researched standardized postmortem protocol to be applied in all cases of sudden unexpected infant and perinatal death.

Anorexia and bulimia nervosa are serious disorders affecting pre-adolescents, adolescents and adults with high levels of co-morbidity, disability and mortality. The medical manifestations of these disorders are intrinsic to the illness and its treatment; some are secondary to malnutrition whereas others are the result of purging or other weight-controlling behaviours and a further group is due to poor management of nutritional problems. This book focuses on the role of the psychiatric, medical, nutritional and psychiatric assessment and management of severely ill patients with anorexia nervosa. Leading clinicians and researchers address the essentials of risk assessment, the identification of complications and the monitoring of treatment progress. Location of care and the role of the team are fully addressed, with due consideration of legal and ethical issues. Practical guidelines are included on risk assessment, management of acute and chronic physical problems, feeding during treatment and consent, as outlined in the MARSIPAN (2010) and Junior MARSIPAN (2012) documents. Critical Care for Anorexia Nervosa will be of value for medical and paediatric teams, GPs, psychiatrists℗land all health-care professionals involved in the treatment of acutely ill patients with℗lanorexia℗lnervosa.

"Current Advances in Osteosarcoma summarizes molecular and genetic characteristics, new therapeutic ideas, and biological characteristics that have been uncovered in this field over the past 10 years. Osteosarcoma is an aggressive malignant neoplasm and is the most common histological form of bone cancer. Osteosarcoma accounts for approximately 56% of new bone tumors, making it the most primary malignant bone tumor in children and adolescents. The lungs are the most common site of metastases and once osteosarcoma spreads to the lungs, it is very difficult to treat. In order to improve the outcome of this disease, the biology of osteosarcoma needs to be better understood. There are numerous investigators around the world who have made seminal discoveries about the important molecular pathways and genetic alterations that contribute to the development and metastases of osteosarcoma. Other investigators have proposed novel therapeutic strategies including some based on the molecular and genetic phenotype of the disease. This volume will provide a comprehensive review of these new discoveries in one singular text, which will help move the field forward."

This book provides readers with a succinct but comprehensive overview of current concepts relating to intestinal failure, including its treatment, with a particular focus on recent advancements. The opening chapters address basic aspects of intestinal failure, including etiology, clinical pathophysiology, physiology of intestinal adaptation, and intestinal microbiota. The complications of intestinal failure, such as bacterial overgrowth and intestinal failure-associated liver disease, are then discussed. A series of chapters specifically address important strategies in the nutritional, pharmacological, and surgical treatment of intestinal failure as well as the concept of intestinal rehabilitation. Finally, two significant emerging developments are considered: regenerative medicine and distraction enterogenesis. The authors are internationally recognized experts from Europe and North America who are ideally placed to describe current knowledge and progress in the field.

The skin is the most accessible organ of the body and also the most frequently injured organ in physical abuse of children. The interpretation of skin lesions, however, is not always simple. In recent years, many reports have been published on pediatric dermatological disorders and accidental injuries that were unjustly regarded as physical signs of child abuse. Knowledge of the differential diagnosis of unexplained skin findings is essential for accurate diagnosis and of vital importance given that errors in either direction (false positive or false negative) can have disastrous consequences.

Over the past 20 years, diagnostic tests for pediatric pulmonologists have revolutionized care of children afflicted with respiratory disorders. These tests have been used to not only help in diagnosis, but also in the management and treatment of these children. Bronchoscopic, imaging and physiologic advances have improved clinical care of these children and have been used as outcome measures in research trials. Diagnostic Tests in Pediatric Pulmonology: Applications and Interpretation describes the various diagnostic modalities (especially the newer ones) that are available for the evaluation of pediatric respiratory disorders. It also provides an understanding of the advantages and limitations of each test so that the clinician may choose the most appropriate ones. An internationally renowned group of authors describe how best to interpret the key findings in a variety of tests as well as the possible pitfalls in incorrect interpretation. This volume focuses on the main diagnostic modalities used in the evaluation of pediatric patients with respiratory disorders and presents up-to-date information on the advantages and limitations of each test for a variety of conditions encountered in the practice of pediatric pulmonology. Clinical utility of these tests is also highlighted. This valuable resource is well suited to practicing clinicians, including pediatric pulmonologists, pediatricians and primary care practitioners, as well as trainees, respiratory therapists and clinical researchers.

Differential diagnosis in pediatric dermatology requires not only a direct experience in the field, but also an inclination to symptoms observations and analysis. This book, the first exclusively devoted to this topic, covers the full range of relevant conditions: inherited skin disorders; nevi; viral, bacterial, fungal, and parasitic infections; acne; allergic diseases; autoimmune skin disorders; connective tissue diseases; tumors; and miscellaneous conditions. Each chapter addresses the differential diagnosis between two (or occasionally three or four) dermatological conditions. All chapters are just two pages long, containing between two and six images and a very short text emphasizing the clinical differences between the diseases in question. At the end of each chapter, a summary highlights one or two characteristics essential for the differential diagnosis. The volume will represent an ideal tool for pediatric dermatologists, pediatricians, and GPs, and will help them in the diagnosic process.

"Efficiency is the key word at the heart of the concept for this diagnostic atlas - efficiency of presentation and diagnostic efficiency achieved through its use. The contents are arranged in chapters according to organ or body part. Each chapter begins with a list of the radiographic findings referring to the relevant table/page number for that finding. The text within these tables is presented under three columns: possible diagnoses, radiographic findings, and general comments. Corresponding sample images of the highest quality are then provided for the more common conditions throughout. Rare diseases are marked with asterisks and are illustrated where appropriate. The hard-pressed radiologist will welcome this work as an efficient and reliable guide to the interpretation of pediatric images"--Provided by publisher.

This book covers the full range of current applications of Doppler sonography in infancy and childhood, describing the variety of potential findings with the aid of a wealth of images. After an introductory chapter on the physical and technical basis of Doppler sonography, applications of cerebral Doppler sonography in infancy and of transcranial Doppler sonography in childhood are addressed, with numerous examples of imaging appearances. The major part of the book is devoted to Doppler sonography of the brain, face and neck and of the abdomen, covering normal abdominal vessels, liver, spleen, pancreas, and mesenteric and renal circulation. Imaging of the ovaries and testes is also presented, encompassing the differential diagnosis of acute scrotum and other space-occupying lesions of the testis. The book closes by considering Doppler sonography of soft tissue and vascular malformations, and the influence of congenital heart malformations on flow parameters in peripheral arteries. Doppler Sonography in Infancy and Childhood will be an invaluable reference for pediatricians, neonatologists, pediatric sonographers, and pediatric and general radiologists.

This book is a practical guide to the radiologic evaluation of acute musculoskeletal injuries of the upper and lower extremities in children. It covers the detection of more subtle and frequently missed fractures and injuries such as buckle fractures, Salter Harris I and II fractures, and epiphyseal and metaphyseal fractures. It also emphasizes the assessment of soft tissues and periarticular fat pads, which can lead to discovery of the sites of bony injuries. Other pathologies that affect the musculoskeletal system, such as infections and tumors, are also discussed. It includes over 600 magnetic resonance, computed tomography, ultrasound, and radiographic images organized by anatomic region.

This book describes different kinds of psychological, physical and sexual maltreatment and explains their possible consequences, especially as regards quality of life, psychopathology and sexual life. The focus is specifically on the victims of abuse, who are among the more frail and vulnerable members ofsociety. The book examines the impacts of maltreatment of both children and social minorities, such as homosexuals and gender dysphoric individuals or those affected by disabilities and describes skills that are of value in supporting victims of maltreatment and preventing discrimination. The work will be useful to sexual medicine specialists, medical doctors, psychologists and psychiatrists, as well as graduate students in these disciplines. By fostering a better understanding of discrimination triggers and effects, it will help clinicians to provide improved support through the tailoring of therapies to the needs of maltreated individuals.
