    Our bones constantly adapt to their mechanical environment through a biological response from the womb to the tomb. Mechanobiology, the biological response to mechanical loading, is important for determining various properties of bone such as size and shape. During embryonic development, rapid growth generates significant tension in the periosteum, and tension has previously been shown to lead to bone apposition. In adults, intracortical stresses dominate and increased loading leads to elevated rates in bone apposition. Periosteal tension and intracortical stresses, then, are both experienced by bones, but their influences on bone apposition rates vary over time. This dissertation analyzes how embryonic bone growth rates and adult bone adaptation rates in long bones are related to their respective mechanical environments. The hypothesis that bones grow and adapt at rates corresponding to changes in the mechanical environment is investigated. In the first study, I investigated the mechanical environment of the periosteum during embryonic growth and its relationship to bone growth rates. The specific growth rate, or percent growth per day, was calculated using microCT images taken over embryonic days 11-20. Bones grew faster in length than in circumference during this time. Finite element techniques were then used to analyze the opening dimensions of incisions through the periosteum. Longitudinal and circumferential residual strains decreased from 46.2% to 29.3%, and 10.6% to 3.9%, respectively, during embryonic days 14-20. Residual strains were positively correlated to specific growth rates (p< 0.05). Many studies have investigated bone adaptation in adult mice and rats by applying loads to the long bones, and measuring changes in periosteal cortical bone apposition rates. However, results are difficult to compare because the loading schemes are generally different. The second study presents a theoretical framework for evaluating the mechanical stimulus based on the bone daily strain stimulus, which is a function of loading cycles and bone strains. The daily strain stimulus may act as a single unifying parameter for directly comparing data from existing in vivo experiments. Two approaches were used to determine the periosteal daily strain stimulus necessary for bone maintenance (xi_peri_0) and the strain-cycle weighting exponent (m), which are required to calculate the daily strain stimulus. In the first approach, data from bone maintenance studies were used to calculate xi_peri_0 to be 2793 microstrain/day, and m to be 4.5. In the second approach, strain gage recordings were used to calculate xi_peri_0 to be 1496 microstrain/day, and human bone compressive fatigue properties were used to assign m to be 11.88. Bone apposition rates generally increased with increasing daily strain stimulus, which was consistent with previous theoretical models. The third study provides examples of how the daily strain stimulus may be used to examine the effects of specific loading parameters on bone apposition rates. The effects of inserting periods of rest and frequency were examined. Inserting periods of rest during loading appeared to increase bone apposition rates by approximately 64% compared to continuously loaded bones. Frequency has been previously suggested to be most osteogenic at 5-10 Hz. Using this analysis, an increase in bone apposition rate was also observed at 10 Hz. The results of these studies provide insight into the effects of periosteal tension during embryonic development and intracortical strains during adulthood on bone apposition rates. These findings illustrate how important the mechanical loads experienced by bones and their surrounding tissues are in determining the sizes and shapes of bones.

    The overall goal of this dissertation is to determine, through a mechanical perspective, how meniscal damage occurs and how subsequent partial meniscectomy can lead to the development of osteoarthritis. Individuals who suffer meniscus injuries suffer both short-term and long-term consequences, from painful joint-locking to premature osteoarthritis. This thesis provides potential mechanisms behind degenerative damage to the posterior horn of the medial meniscus. Once this damage has occurred, this thesis provides an explanation for the changes that occur during walking after partial medial meniscectomy and then suggests a mechanism for why these subjects are at risk for developing premature osteoarthritis. Patterns of regional variation in transverse plane meniscal displacement during positions representative of normal gait were studied using cadaveric knees in a test bed developed to be compatible with magnetic resonance imaging (MRI). The results demonstrated that during the stance phase of walking, the lateral meniscus appears to displace with the femoral condyle while the medial meniscus, especially at the posterior horn, displaced minimally. The relatively low movement of the medial posterior horn suggests that this region sustains substantial cyclic stress to restrain the normal posterior displacement of the medial femoral condyle at heel strike of each gait cycle. These results help explain the chronic patterns of damage to the posteromedial horn. Gait analysis of subjects who had undergone partial medial meniscectomy revealed that during stance phase, these subjects had tibias that were externally rotated by 3.2° compared to their healthy contralateral limbs. Their operated limbs also exhibited reduced peak flexion and extension moments compared to their healthy limbs. The altered rotational position found likely causes changes in tibiofemoral contact during walking and could cause the type of degenerative changes found in the articular cartilage following meniscal injury. A joint level numerical model was developed to look further at the results from the first two studies. This 3D FE model looked at the effects of the previously noted tibial rotational shift and an increased adduction moment on the meniscal strains during a simulated portion of the gait cycle. The increased adduction moment resulted in increased strains of both the anterior and posterior horn of the medial meniscus. The external tibial rotational shift resulted in decreased strains in the posterior medial horn and increased strains in the anterior medial horn. The lateral meniscus exhibited much lower strains than the medial meniscus and minimal changes under the various loading conditions. It appears that the shift towards external tibial rotation seen clinically after partial medial meniscectomy is not likely to cause medial meniscal damage and therefore, the kinematic shift might be a result of the surgery. Finally, the FE model was then used to look at the effects of varying levels of partial medial meniscectomy on the rotational resistance of the tibia during a portion of the stance phase of gait. Additionally, the stresses and contact pressures in the articular cartilage of the knee were determined. The results showed that the more meniscal tissue that was removed, the greater the loss of rotational stability that was provided by the geometry. Additionally, the location of removal of meniscal tissue was important as the central rim resulted in a greater loss of rotational stability than removal from the anterior or posterior horns. Similarly, the more meniscal tissue that was removed, the higher the increase in contact pressures and stresses within the articular cartilage. Thus, the results indicate that it may simply be the resulting change in meniscal geometry after partial medial meniscectomy that results in the kinematic shift towards external tibial rotation we see in individuals who have undergone partial meniscectomy. The combination of the change in contact location along with the potential increase in cartilage stresses seen post-meniscectomy may place these individuals at higher risk for developing premature osteoarthritis. Taken together, the results of these studies provide potential mechanisms for the development of damage to the medial meniscus and then the subsequent changes that may lead to the development of premature knee osteoarthritis after partial meniscectomy. Additionally, the insights provided by this work have direct implications into treatments for meniscal injuries in order to prevent degenerative changes in articular cartilage.

    In recent years, mini-invasive surgery has become increasingly important for reducing the risk of infection and minimizing blood loss and volume of implants. Hip surgery requires small incisions, which make the use of the appropriate equipment and an extensive knowledge of the anatomy of the region essential. Mini-invasive surgery requires the surgeon to consider the indications for surgery for each patient he treats and to know the exact loco-regional anatomy for that patient. Methods must be very precise and warrant visual explanations to help teach young surgeons.

    Contemporary spinal surgeons, whether orthopedic or neurosurgeons, are increasingly recognizing minimally invasive spine surgery (MISS) as a valuable option for managing advanced degenerative diseases.     Bone and the immune system are both complex tissues, which, respectively, regulate the skeleton and the body's responses to invading pathogens. It has become increasingly clear that critical interactions between these two organ systems frequently occur. This is particularly true for the development of immune cells in the bone marrow and for the function of bone cells in health and disease. However, because these two disciplines developed independently, investigators in each don't always fully appreciate the significance of the other. This book is meant to provide a detailed overview of the many ways that bone and immune cells interact. The goal is to provide basic and clinical scientists with a better understanding of the role that the immune system and bone play in the development and function of each other so that advances in both fields will be facilitated. The focus of the book will be both on basic pathways and translational science, which will apply basic knowledge to clinical diseases. Chapter content will range from basic descriptions of the various cell systems and their development to the signals that cause them to interact during normal physiology and disease. Approximately half the book will focus on the role that osteoimmunology plays in the development of human diseases.

    The book presents a problem-based, rational approach to the diagnosis of orthopaedic problems in children. It is divided into five sections based on the age at which specific problems may be encountered; the newborn, the neonate and infant, the toddler and the pre-school child, the child in the school-going age and the adolescent. The authors outline a systematic approach to arrive at a tentative diagnosis by asking relevant questions related to the presenting symptom and then performing a thorough clinical examination. They then suggest the most appropriate investigations to confirm the diagnosis. Each chapter deals with the common conditions that may lead to the problem; the rarer causes of the problem are listed at the end.

    MRI is the only imaging technique that allows direct visualization of bone marrow and its components with good spatial and contrast resolution. Accordingly, it is an optimal imaging modality for bone marrow assessment in both adults and children. This comprehensive atlas is unique in being devoted to the MRI appearances of bone marrow in the axial and appendicular skeleton of adults and children. Normal MRI findings, including common variants and degenerative changes, are first documented. MRI appearances in the entire spectrum of neoplastic and non-neoplastic infiltrative marrow disorders are then presented, with accompanying explanatory text. Among the conditions considered are multiple myeloma, the acute and chronic leukemias, diffuse metastases, diffuse lymphomas, the anemias, polycythemia vera, myelofibrosis, storage disorders, and infections. Characteristic changes to bone marrow following various forms of treatment are also displayed and discussed. The selected images reflect the use of a variety of sequences and techniques, such as fat suppression, and contrast-enhanced imaging.
