While advances in CLL therapy are noted, many patients still succumb to this illness. Like most progress in medicine, solid advances in the diagnosis, prognosis and treatment of CLL are rooted in an in-depth understanding of the basic and translational biology of CLL. In this book, CLL experts have contributed state-of-the-art summaries of various important aspects of CLL biology and have discussed the translational implication of such findings. - 2015 SpringerAlessandro Plebani, Vassilios Lougaris, editors.This book provides an updated overview of agammaglobulinemia, a rare form of primary immunodeficiency which is considered the prototype of the congenital humoral defects, and which is characterized by the absence of peripheral B cells and very low serum immunoglobulin levels. The book opens by discussing the highly orchestrated early B cell development in the bone marrow and the genes involved based on both human and animal models. The pathogenesis and clinical presentation of X-linked agammaglobulinemia, caused by mutations in the BTK (Bruton's tyrosine kinase) gene, are then presented in detail, followed by descriptions of the clinical manifestations and molecular basis of the less frequent autosomal recessive and autosomal dominant forms of agammaglobulinemia. Patients' management in terms of respiratory complications, gammaglobulin replacement therapy and the potential value of novel experimental therapeutic strategies are discussed. The book's closing chapters offer a comprehensive and updated description of mutations in the BTK gene, and the expression and function of BTK in cells other than B cells.

In total, 940 cases of CRS were diagnosed in this population and these patients were subjected to detailed analysis. The opening chapters address the definition and classification of CRS, epidemiology and pathogenesis, covering molecular and cellular mechanisms, radioadaptation, and the role of tissue reactions. The pathoanatomy of CRS during the development and recovery stages is discussed for all organ systems. Hematologic malignancies have recently been identified among the obesity associated malignancies spanning the lifespan from childhood to the elderly and include leukemia, myeloma, lymphoma and others. In addition to the etiologic association between obesity and hematologic malignancies, the presence of obesity has profound effects on therapy by impacting pharmacokinetics of chemotherapeutic agents, dose, adipocyte metabolism and drug distribution. These may be particularly important in hematopoietic stem cell transplantation. Another important aspect of the association of obesity with hematologic malignancies is the increased incidence of obesity in children who successfully complete therapy for acute lymphoblastic leukemia. This and other observations indicate important relations between the hematopoietic systems and fat metabolism. This volume on Energy Balance in Hematologic Malignancies will provide an important volume in this series and a basis for better understanding etiology, mechanisms, therapeutics implications and experimental approaches. This volume of energy balance and cancer will focus on the relation of obesity to hematologic malignancies, the epidemiology, potential mechanisms, and thereapeutic considerations including effects on pharmacologic and physical approaches as well as the delayed effects of therapy on energy balance.

- Gaining access to the hematopoietic stem cell niche : novel non-myeloablative conditioning approaches2010Agnieszka Czechowicz.Sickle Cell Anemia, Beta Thalassemia, Severe Combined Immunodeficiency (SCID), and Type I Diabetes share one commonality: these diverse disorders can all be attributed to faulty hematolymphoid effector cells which are largely caused by genetic mutations that alter hematopoietic cell-intrinsic function. These defective blood cells inherit their genetic deficiencies from hematopoietic stem cells (HSC) as they differentiate. Thus, each of these unique diseases should be theoretically curable through the same strategy: replacement of patients' HSCs carrying the problematic mutation with normal HSCs from disease-free donors, thereby generating entire new, healthy hematolymphoid systems. Replacement of disease-causing stem cells with healthy ones has been achieved clinically via hematopoietic cell transplantation (HCT) for the last 50 years, as a treatment modality for a variety of cancers and immunodeficiencies with moderate, but increasing success. However this technique bears high morbidity and mortality severely limiting its utilization. Purified hematopoietic stem cell transplantation is associated with high toxicity primarily attributed to the myeloablative pre-conditioning necessary to facilitate engraftment of the donor HSCs. However the function and mechanistic role of these reagents has been up until now poorly understood. This thesis deconstructs the current barriers in HCT and pure HSC transplantation, and suggests that in addition to the immune barrier, hematopoietic stem cell niche availability is another critical obstacle that needs to be overcome to allow for efficient hematolymphoid replacement. In this thesis I show that in the homeostatic state, most hematopoietic stem cell niches are occupied by host HSCs limiting the ability of incoming HSC engraftment. However, a limited number of HSC niches are vacated by HSC that are egressing from the peripheral blood into the bone marrow in a division independent manner, allowing for true but minimal HSC engraftment without myeloablative pre-conditioning. This thesis illustrates new, up-and-coming strategies to combat the obstacle of HSC niche availability. Here I prove that one non-myeloablative conditioning approach that capitalizes upon physiological HSC egress is repetitive transplantation of purified HSCs. However, this approach is rather laborious and inefficient, and I show that another more efficient novel non-myeloablative conditioning approach to vacate the HSC niche is antibody-mediated depletion of host HSCs. Furthermore, in this thesis I demonstrate that antibody-mediated inhibition of c-kit effectively depletes host HSCs and allows for efficient, non-toxic HSC transplantation. These novel non-myeloablative conditioning methods illustrate new means of overcoming the current barriers to hematolymphoid replacement thereby allowing for the potential expansion of this technique to treat a variety of inherited diseases of hematopoietic and immune function.

This book recognizes the importance of accurate and speedy interpretation of symptoms in that the text is symptom oriented rather than disease oriented. Put another way, the reader confronted with a particular clinical problem or symptom will be guided step by step to the possible underlying disorder(s). The scenarios considered include critical illness in patients with anemia, leukopenia, leukocytosis, abnormal platelet count, and severe transfusion reactions. For each circumstance, factors relevant to symptom interpretation are fully discussed. In addition, helpful diagnostic algorithms are presented in the form of flow charts that will assist in decision making regarding the need for further investigations. The closing chapter is devoted to the drug-induced hematologic disorders. Although the book is intended mainly for intensivists, other specialists will find much information of value for their daily practice.
